Summary
This study tests a program called SHIFT2 that helps teens and young adults with type 1 diabetes prepare to move from pediatric to adult medical care. About 109 participants, aged 16-22, and their caregivers will take part. The goal is to see if the program improves blood sugar control, diabetes self-care, and clinic attendance during this important transition.
