Summary
This study is testing whether a special homecare program can help people recover better after a stroke, especially those facing social challenges like poverty or unstable housing. The program sends a team including a nurse and a social worker to visit patients at home after they leave the hospital. Researchers will compare recovery, blood pressure, and thinking skills between people who get these visits and those who receive usual care.
