Summary
This study is testing whether a homecare program can help people recover better after a stroke, especially if they face social challenges like unstable housing, low income, or lack of support. The program sends a team including a nurse, social worker, and community health worker to visit patients at home after they leave the hospital. Researchers will compare recovery, blood pressure, and thinking skills in people who get these visits versus those who receive usual care.
