Summary
This study tests two ways to follow up with adults who survived sepsis in the hospital. About 1,410 patients in four African countries will get either an enhanced discharge plan or that plus automated phone check-ins. The goal is to see if these strategies reduce deaths within 90 days after leaving the hospital.
