Summary
This study tested a nurse-led disease management program using a mobile app and telehealth for people with coronary artery disease at high risk of returning to the hospital. 101 participants were split into two groups: one received the program plus usual care, the other usual care alone. The goal was to see if the program could reduce unplanned hospital readmissions or death.
