Summary
This study tested a nurse-led disease management program using a mobile app and telehealth for people with coronary artery disease at high risk of returning to the hospital. 101 participants were split into two groups: one received the program plus usual care, the other usual care alone. The goal was to see if the program could reduce hospital readmissions or death by helping patients manage their condition better.
