Can a smartphone app improve end-of-life care for heart failure patients?

Summary
This study looks at whether a remote monitoring system, called RAPHAEL, can work for heart failure patients who may need palliative care. About 90 adults with worsening heart failure will fill out a digital questionnaire for 4 weeks, and their healthcare team will follow up as usual. The goal is to see if patients and doctors find this approach easy to use and helpful, not to test any new drugs or devices.
