Can a simple digital Check-In improve heart failure care?

Summary

This study looks at whether a remote monitoring system (called RAPHAEL) can work for adults with heart failure who may need palliative care. About 90 patients will answer digital questionnaires about their symptoms and needs over 4 weeks. Doctors will review the responses and follow up as usual. The goal is to see if this approach is practical and acceptable, not to test a new drug or device.
