Summary
This study will test a program that combines peer support with remote patient monitoring to help Black adults with uncontrolled type 2 diabetes. Researchers will enroll 200 participants from primary care clinics in Alabama and track changes in blood sugar (A1C) and blood pressure over six months. The goal is to see if this approach improves diabetes management and can be easily used in real-world clinics.
