Summary
This study tests a program called PIC-TRFS to help patients aged 50 and older recover at home after a stay in the intensive care unit (ICU). The program includes health management, rehabilitation, social support, and care coordination through telehealth. Researchers will compare the quality of life and functioning of those in the program to those receiving usual care over 12 months. The goal is to see if this support improves recovery and reduces caregiver burden.
