Summary
This study is testing whether portable medical devices like ultrasound scanners and blood glucose monitors can help doctors detect chronic diseases earlier in regular doctor's offices. About 200 adults at risk for conditions like diabetes, heart disease, and kidney problems will either use these portable devices alongside their normal care or receive standard care alone for six months. Researchers want to see if these easy-to-use tools can spot health problems sooner than traditional methods.
