Summary

This study tests whether a pharmacist-led tele-education program can help adults with uncontrolled type 2 diabetes improve blood sugar control, diabetes knowledge, medication adherence, and quality of life. About 154 participants will either receive five virtual education sessions with a pharmacist or continue standard care. The program is delivered via phone or Zoom and aims to support patients in managing their condition long-term.
