Summary
This study tests whether a pharmacist-led tele-education program (PharmaTE) helps adults with uncontrolled type 2 diabetes improve blood sugar control, diabetes knowledge, medication adherence, and quality of life. About 154 participants will either receive five virtual sessions with a clinical pharmacist or continue standard care. The program is designed to be convenient and accessible, using phone or Zoom calls.
