Summary
This study looked at whether having a pharmacist check in with older Veterans via video call at home could improve how they take their medications. The study included 304 Veterans who had at least two chronic conditions and took five or more daily medicines. The goal was to reduce the use of potentially harmful medications and improve overall medication safety.
