Summary

This study looked at whether a pharmacist-led medication review done through a video call to the home could improve medication safety for older Veterans. The study included 304 Veterans who take at least 5 daily medications and have 2 or more chronic conditions. The goal was to reduce the use of potentially harmful medications and improve how well patients understand and manage their medicines.
