New tool aims to bring comfort care to heart failure patients sooner

Summary
This study will test a new computer tool that helps doctors decide when to refer people with heart failure to palliative care. The tool uses a risk model to predict who might benefit from extra support. Researchers will see if the tool is easy to use and if it leads to more timely referrals. About 2,260 patients and their doctors will take part.
