Summary
This study is testing whether a new patient support program can help prevent future bone fractures in people who have already broken a bone due to osteoporosis. The program connects patients with a navigator who helps them get to bone health specialists, and compares this approach to standard education for patients and their doctors. Researchers want to see which method works better at preventing another fracture over two years, and if it works equally well for people of different ages, races, and backgrounds.
