Summary
This study tests a program called Pediatric Medication Therapy Management (pMTM) for children ages 2-18 with complex medical conditions who take 5 or more medications. The goal is to see if the program helps doctors and families find and fix medication-related problems, such as drug interactions or unnecessary pills. About 371 children from Children's Hospital Colorado will take part.
