Summary
This study tested whether a standardized electronic note could improve communication and reduce medical errors when patients are transferred from one hospital to another. Over 1,600 adults admitted to general medicine, cardiology, oncology, or ICU services participated. Researchers measured errors and adverse events to see if the tool made transfers safer.
