Summary
This study tested a simple tool—a standardized electronic note—to improve communication when patients are moved from one hospital to another. Over 1,600 adults transferred to a large hospital's medical, heart, cancer, or ICU units took part. The goal was to see if the note reduced medical errors and made transfers safer.
