New note system cuts errors in Hospital-to-Hospital transfers

This study tested a simple tool—a standardized electronic note—to improve communication when patients are moved from one hospital to another. Researchers tracked medical errors and safety events in over 1,600 adults transferred to a large hospital. The goal was to see if better information sharing could reduce mistakes and improve patient safety.
