Nurse-Led teams may cut hospital returns for patients with multiple illnesses

Summary
This study looks at whether a nurse-led team can help adults with two or more chronic diseases (like heart failure, diabetes, or kidney disease) get the right medications during a hospital stay. About 178 participants will either receive this extra team support or usual care. Researchers will track if the nurse-led approach leads to better use of recommended drugs and fewer hospital visits or deaths within 90 days after discharge.
