Could a gut bacteria transplant ease a rare lung disease?
NCT ID NCT07751120
First seen Aug 07, 2026 · Last updated Aug 14, 2026 · Updated 2 times
Summary
This early-stage trial tests whether transplanting gut bacteria from healthy donors, along with antibiotics and fiber, is safe and feasible for people with pulmonary arterial hypertension (PAH), a condition of high blood pressure in the lungs. About 24 adults with PAH will be randomly assigned to receive the bacteria capsules with fiber, bacteria with a placebo, or placebos only. Researchers will track how well the donor bacteria take hold in the gut, monitor side effects, and look at early signs of impact on heart function, walking ability, and quality of life.
What this could mean
Our plain-language read of the trial. This is informational only, not medical advice or a prediction.
- Active substance
- Microbiota transplant therapy (freeze-dried gut bacteria from healthy donors) with antibiotic preconditioning and fiber supplementation
- What this could lead to
- If successful, this could pave the way for a new gut-based treatment for pulmonary arterial hypertension, potentially improving symptoms and quality of life.
- What could go wrong
- This is an early, small pilot study focused on safety and feasibility, not proof of effectiveness. The transplant may not engraft or provide clinical benefit, and there are risks from antibiotics and the transplant itself.

Inclusion Criteria: * Provision of signed and dated consent form * Ages 18-75 * Diagnosis of PAH * On stable treatment for PAH for one month prior to enrollment * Able to swallow capsules * Able to provide blood sample and fecal sample * Stated willingness to comply with all study procedures and availability for the duration of trial to follow-up by telephone, in-person, email, and/or video visits or correspondence. Exclusion Criteria: * Dysphagia to pills * Clinically active inflammatory bowel disease * Pregnancy or breastfeeding. A pregnancy test will be obtained from females of child- bearing potential at the screening visit or day 1 (prior to the receipt of MTT). Patients with a positive pregnancy test will be excluded. A negative result will be required for subjects who are females of child-bearing potential to receive MTT. Patients will be counseled to avoid pregnancy which is the standard of care for patients with PAH. * Life expectancy of \< 6 months * Presence of ileostomy or colostomy * Patients on immunosuppressants (calcineurin inhibitors, prednisone ≥ 20 mg/day, methotrexate, azathioprine, immunosuppressive biologics, JAK inhibitors). * Patients with neutropenia (an absolute neutrophil count \< 0.5 x 10 9 cells/L)
