First seen May 27, 2026 · Last updated Jun 19, 2026 · Updated 4 times

Summary

This study tests whether a support program led by a nurse and community health worker can help people with type 2 diabetes manage their medications after leaving the hospital. The program uses an AI tool called MEDBRIDGE to identify high-risk patients and provides 3 months of follow-up care. The goal is to see if the program is practical and acceptable, and to track changes in blood sugar and emergency visits. Forty-five patients will take part at two Alabama hospitals.

What this could mean

Our plain-language read of the trial. This is informational only — not medical advice or a prediction.

Active substance

MEDBRIDGE-Guided NCM/CHW Post-Discharge Support Intervention

What this could lead to

If this works, it could point toward a practical way to help people with diabetes avoid medication mistakes and hospital readmissions after discharge.

What could go wrong

This is a small, early feasibility study with only 45 participants, so results may not apply to everyone. The program may not improve blood sugar or reduce emergency visits as hoped.
