Summary
This study tests whether a support program led by a nurse and community health worker can help people with type 2 diabetes manage their medications after leaving the hospital. The program uses an AI tool called MEDBRIDGE to identify high-risk patients and provides 3 months of follow-up care. The goal is to see if the program is practical and acceptable, and to track changes in blood sugar and emergency visits. Forty-five patients will take part at two Alabama hospitals.
