Summary

This study tests whether letting patients with chronic kidney disease manage their own blood pressure medications, with guidance from a pharmacist, works better than standard care. About 160 veterans with kidney disease and high blood pressure will take part. The goal is to see if this self-management approach leads to better blood pressure control.
