Summary
This study tests a new way of organizing care for chronic diseases like high blood pressure, diabetes, and kidney disease in Thailand. Researchers will work with 16 primary care clinics to see if using patient data in real time helps doctors and nurses provide better care. The goal is to improve health outcomes and reduce inequalities, not to cure the diseases.
