Summary
This study tests a program to help doctors better screen and treat chronic kidney disease (CKD) in adults with type 2 diabetes. About 420 participants will receive care through their primary doctor, with the goal of increasing use of recommended medications. The program focuses on improving early detection and management to slow kidney damage.
