Can fasting patterns rewire metabolism to halt prediabetes?
NCT ID NCT07755982
First seen Aug 10, 2026 · Last updated Aug 11, 2026 · Updated 1 time
Summary
This trial tests whether different intermittent fasting schedules—5:2, alternate-day, and time-restricted eating—can improve blood sugar, cholesterol, inflammation, and body composition in adults with prediabetes. Participants will be randomly assigned to one of three fasting plans or a control diet for 4-6 weeks. The study also looks at how genes may influence individual responses to fasting. The goal is to find which fasting approach offers the most metabolic benefit for people at risk of type 2 diabetes.
What this could mean
Our plain-language read of the trial. This is informational only, not medical advice or a prediction.
- Active substance
- Intermittent fasting (three patterns: 5:2, alternate-day, and time-restricted eating)
- What this could lead to
- If successful, this could identify which intermittent fasting pattern best improves blood sugar, cholesterol, and body composition in people with prediabetes, potentially offering a practical dietary strategy to prevent type 2 diabetes.
- What could go wrong
- This is a short-term trial (4-6 weeks) with a moderate sample size, so results may not be durable or generalizable. Adherence to fasting can be challenging, and individual responses may vary.

Inclusion Criteria: 1. Patients with prediabetes, defined according to ADA criteria (fasting plasma glucose 100-125 mg/dL or 2-hour postprandial glucose 140-199 mg/dL). 2. Willing to follow the fasting/diet intervention protocol for 4-6 weeks (depending on the study duration). 3. Signing informed consent and willing to be randomized into one of 3 intervention groups (Monday-Thursday Fasting/5:2, Alternate-Day Fasting/ADF, or Time-Restricted Fasting/IF). Exclusion Criteria 1. Diagnosis of type 2 diabetes mellitus. 2. Currently undergoing antidiabetic drug therapy (e.g., metformin, sulfonylurea, insulin). 3. Has a severe chronic disease (kidney failure, liver failure, heart failure, or eating disorder). 4. Pregnancy or breastfeeding. 5. History of eating disorders (anorexia nervosa, bulimia nervosa, or binge eating disorder). 6. Taking medications that affect glucose metabolism (e.g., systemic steroids, atypical antipsychotics, etc.). Diagnosis of type 2 diabetes
