Summary
This study tests whether a one-year program led by heart failure nurses, using online training and a mobile app, can reduce hospital readmissions and improve quality of life for people hospitalized with heart failure. About 200 patients will either receive the program or standard care. The program includes education, monitoring, and connections to specialists and exercise groups.
