First seen Nov 01, 2025 · Last updated Apr 30, 2026 · Updated 27 times

Summary

This study looks at whether a patient navigator or a website (or both) can help people who have had a stem cell transplant for blood cancer plan their future medical care. About 40 adults who are 1 to 5 years post-transplant will take part. The goal is to see if these tools help them have important conversations with their doctors and complete advance directives.
