New study aims to help transplant survivors have crucial End-of-Life conversations

Summary
This study looks at whether a patient navigator or a website called PACT can help people who had a stem cell transplant for blood cancer plan their future medical care. About 40 adults who are 1 to 5 years post-transplant will be enrolled. The goal is to see if these tools help patients have important conversations with their doctors and complete advance directives.
