Summary
This study tested a new way to help people with two or more long-term health problems like HIV, high blood pressure, diabetes, asthma, or depression. Over 1,800 adults aged 40 and older in South Africa took part. Some clinics used a special decision-support tool and home visits from health workers, while others used standard care. The goal was to see if the new approach could better detect, treat, and control these conditions.
