Summary
This study looks at whether a questionnaire called AT HARM 10 can help identify emergency room visits caused by medication problems in adults over 65. Researchers will test the tool on 100 patients at one hospital to see how many drug-related issues they can find. The goal is to improve treatment and prevent future hospital visits.
