Summary
This study tests a simple questionnaire called AT HARM 10 to help identify when a hospital visit is caused by a medication problem. Researchers will enroll 100 adults over 65 in the emergency department to see how well the tool works. The goal is to improve treatment and prevent future drug-related hospital stays.
