Summary
This study looks at whether a hybrid care model—using digital tools like telemonitoring and online consultations—can help manage type 2 diabetes and chronic heart failure during health emergencies. About 180 patients will be observed and interviewed over a year to see how this approach affects their experiences and outcomes. The goal is to make healthcare more resilient without adding physical risks.
