Summary

This study looks at two different levels of support for children with complex health needs when they leave the hospital. About 480 families will take part to see which approach reduces emergency visits and readmissions, and helps parents feel more confident caring for their child at home. The goal is to find the best way to help these children transition safely from hospital to home.
