Summary
This study looks at whether a personalized home exercise program can improve heart and kidney function in adults with cardiorenal syndrome (combined heart and kidney problems). Researchers will measure changes in exercise ability, kidney function, and heart function to see if exercise is safe and effective for managing this condition.
