Can a home BP monitor and a health coach keep your blood pressure in check?

Summary
This study tests a program that combines home blood pressure monitoring with support from community health workers to help adults at risk for high blood pressure. About 600 adults with slightly elevated blood pressure will either get a home monitor and usual care, or the full program including training, a phone app, and health coaching. The goal is to see if this approach can prevent blood pressure from rising to dangerous levels over 12 months.
