New program aims to help older veterans get back on their feet after hospitalization

Summary
This study is developing and testing a program to help older veterans recover their physical function and independence after leaving the hospital. About 40 veterans aged 65 and older with some mobility challenges will take part. The program adds special assessments and support to an existing VA transitional care service to see if it helps veterans regain strength and improve their quality of life.
