Summary
This study tests whether a special clinic that helps patients with heart failure after they leave the hospital can reduce the chance of being readmitted. About 150 adults aged 18 to 90 will get extra education, tools like a blood pressure monitor, and follow-up phone calls. The goal is to help patients manage their condition at home and avoid returning to the hospital.
