Summary

This study tests whether a clinic that provides extra education, tools, and follow-up calls can reduce hospital readmissions for people with a type of heart failure called HFpEF. About 150 adults discharged from Ocean University Medical Center will either receive this extra support or standard care. The goal is to help patients manage their condition at home and improve their quality of life.
