Summary
This study is testing a new screening and alert system to prevent heart failure in people with type 2 diabetes. Doctors' clinics are randomly assigned to either use or not use a tool that calculates a patient's heart failure risk. If a patient is flagged as high-risk, their doctor gets an alert with recommendations to start specific preventive medications.
