Summary
This study aims to prevent heart failure in adults at high risk, primarily due to high blood pressure, in Detroit and Cleveland. It uses mobile health units to reach people in their communities and pairs them with community health workers and pharmacists to improve access to care and optimize medications. The goal is to see if this approach lowers blood pressure and other risk markers better than usual care over 12 months.
