Summary
This study is testing whether a one-year digital support program can help people recently hospitalized for heart failure stay healthier at home. The program connects patients with specialized heart failure nurses through an app and online learning, providing education and monitoring. Researchers want to see if this approach reduces return trips to the hospital and improves patients' quality of life compared to standard care.
