Summary
This study is testing whether a one-year, web-based support program guided by a specialized heart failure nurse can help people recently hospitalized for heart failure. The program includes online education, an app for tracking health, and connections to support networks. Researchers want to see if this extra support leads to fewer repeat hospital stays and a better quality of life compared to standard care.
