Summary

This study is testing whether a 6-month support program can help prevent heart failure in adults with type 2 diabetes, obesity, and low incomes. The program combines exercise training at local YMCAs, problem-solving coaching for managing health, and support from community health workers. Researchers want to see if this approach improves fitness, blood sugar control, and heart health markers better than just receiving educational materials and access to an exercise facility.
