Summary
This study tests a new approach to help Black children and young adults with type 1 diabetes use advanced diabetes tools like continuous glucose monitors and insulin pumps. Participants will use a smartphone app and get support from a community health worker, while their care team uses a special computer dashboard. The goal is to see if this program can reduce racial disparities in diabetes technology use.
