This study looks at whether an electronic comprehensive geriatric assessment (eCGA) can be easily used in primary care for adults aged 65 and older. Researchers will track frailty over 12 months and compare patient self-reports with doctor assessments. A smaller group will also test a personalized exercise program. The goal is to find better ways to support healthy aging in routine care.
