Summary
This study looks at whether an educational discussion guide helps postpartum patients who had high blood pressure during pregnancy become more active in managing their health. About 150 new mothers will either receive the guide shortly after delivery or not, and their engagement will be measured through surveys 4-12 weeks later. The goal is to see if the tool improves patient activation, not to treat the condition directly.
