This study looks at whether an educational discussion tool helps postpartum patients who had high blood pressure during pregnancy become more involved in their own care. About 150 new mothers will either receive the tool shortly after delivery or not, and researchers will compare their engagement scores weeks later. The goal is to see if the guide improves patient activation, not to treat the condition directly.
